Does the referring dentist need a Sindi account to submit a referral?
No. Any general dentist can submit a referral through your embedded form or shareable link without creating a Sindi account. They fill out the form, attach any files, and hit submit. That's it. The referral lands in your Sindi dashboard immediately. When you accept and schedule the referral, the sender will get automated updates as the case progresses so they stay in the loop.

Can I build my referral form from scratch, or am I limited to templates?
Both options are available. Every Sindi form starts with a set of standard fields (patient demographics, insurance, clinical notes) that are always included. From there, you build your custom questions entirely from scratch — create any question, name it whatever you want, choose from 6 field types, and mark each one required or optional. There are no templates you can't change.

What field types are available for custom questions?
Six types of questions so you can ask anything: single select (choose one from a list), multiple choice (choose any that apply), yes/no, text field (open-ended answer), date (with date picker), and dropdown list. Each can be marked required or optional, and you can add as many custom questions as your workflow needs.

How does the referral get to me once it's submitted?
It appears immediately in your Sindi referral dashboard as a structured record — patient information pre-populated, custom question answers filled in, and any attached files included. You'll receive a notification by email and in-app. From there you can accept, assign to a provider, and begin tracking the case — all in one place.

Can referring dentists attach x-rays and patient files to the form?
Yes. The Sindi referral form includes a file attachment step where referring dentists can attach x-rays, patient photos, and any clinical documents before submitting. Files arrive with the referral — you don't have to chase them down separately.

Is the referral form HIPAA-compliant?
Yes. All submissions through Sindi referral forms are transmitted and stored with HIPAA-compliant security — including any attached patient files. A Business Associate Agreement (BAA) is available for all practices.

How does the embed work on my practice website?
Sindi generates a code snippet you paste into any page on your practice website. It renders as a fully styled form that matches the referral experience — no separate login or redirect required. If you prefer not to embed it, Sindi also provides a shareable direct link you can send to any referring office or post wherever you like. It usually takes a practice's website manager about 5 minutes to embed. We're happy to work directly with them to get the form live on your site.
